- There is a one-off Administration fee of £9.50 per adult (Not chargeable to Children’s Plans) which will be collected along with your first monthly Direct Debit Collection.
- Monthly payment will be collected on or around the 8th of every month by Practice Plan Ltd and the collection date cannot be changed.
- You will have instant access to the Worldwide Dental Emergency Assistance Scheme and Emergency assessment appointments at the practice upon completion of registration.
- You will be entitled to use the appointments and discount covered by your plan level after the 6th monthly payment has been received.
- You will not be asked to sign a contract for your membership, so you can cancel this at any time by contacting Practice Plan Ltd on 01691684120 or by cancelling your Direct Debit with your bank.
- Patients are responsible for making and attending appointments provided by your Plan Membership.
- Any Entitlements that are not used in any given period may not be carried forward or refunded.
